Free Trial Class Booking Policy Welcome to Stephen Gaughan SOPA! We are thrilled to offer you a free trial class to experience our training first-hand. To ensure a safe and productive environment for all students, please review our booking requirements below. 1. Required Information To secure your trial spot, the booking form must be completed in full. We require the following details for insurance and safeguarding purposes: Full Name (Student and Parent/Guardian if under 18) Home Address Telephone Number Email Address Emergency Contact Details (Name and Phone Number) Health Conditions & Allergies: You must disclose any medical conditions, injuries, or allergies. 2. Health & Safety Disclosure Your safety is our priority. Please note that Stephen Gaughan SOPA cannot take responsibility for any incidents, injuries, or allergic reactions resulting from information that was not disclosed prior to the start of the class. > Important: It is the responsibility of the participant (or parent/guardian) to ensure all medical information provided is accurate and up to date. 3. Cancellation of Booking We take pride in our small class sizes and high-quality instruction. Because spaces are limited: Incomplete Forms: Failure to submit all required information may result in your booking being automatically cancelled without notice. Notice of Absence: If you cannot attend your scheduled trial, please notify us at least 24 hours in advance so we can offer the spot to another student. Agreement By submitting your booking form, you acknowledge that you have read, understood, and agreed to the terms above. We look forward to seeing you in the studio!
